About Our Services
What services do you provide?
We offer NDIS Specialist Behaviour Support, Psychology & Counselling, and Clinical Supervision for practitioners. We also provide consulting, training, and clinical governance support for organisations.
Do you only work with NDIS participants?
Most of our work is with NDIS participants, but we also provide private psychology services and professional supervision outside of the NDIS.
Do you provide telehealth?
Yes. We offer telehealth across most of our services. This can be a flexible option if you live regionally or prefer to connect online.
NDIS & Funding
Are you a registered NDIS provider?
Yes. Forming Connections is a registered NDIS provider. We deliver services under Capacity Building – Improved Relationships (for behaviour support) and Capacity Building – Daily Living (for psychology).
Do you support both self-managed and plan-managed participants?
Yes. We currently support self-managed and plan-managed participants. We do not invoice the NDIA directly for agency-managed plans.
Which line items are your services billed under?
- Positive Behaviour Support → Improved Relationships line items.
- Psychology → Improved Daily Living line items.
- Supervision and consulting → Invoiced directly to you or your organisation.
How to Endorse Forming Connections
The NDIA is moving everyone onto their PACE platform. As a participant, plan nominee or decision maker, only you have the authority to endorse a service provider to the NDIS.
We would like to work with you and so we’ve made the below step by step process to help you know what you will need to tell the NDIS.
Follow these steps below: Call the NDIS on 1800 800 110 and tell them you would like to endorse Forming Connections as your service provider.

Referrals & Intake
How do I make a referral?
You can make a referral by completing our online form, emailing us, or giving us a call. Support Coordinators and Plan Managers are welcome to submit referrals on behalf of participants.
What happens after I make a referral?
Our intake team will contact you to gather more details, confirm funding, and discuss your goals. We then send a Service Agreement before services begin.
Do you have a waitlist?
Sometimes. We’ll let you know at referral if there is a wait, and we’ll offer estimated timeframes.
Appointments & Delivery
Where do sessions take place?
We offer sessions:
- in the participant’s home
- in schools or community settings (with consent)
- via telehealth
How long are sessions?
Session length varies depending on the service. Most therapy sessions are around 50 minutes. Behaviour support sessions may be longer when assessments or observations are required.
Do you provide reports or behaviour support plans?
Yes. We prepare reports, functional behaviour assessments, and behaviour support plans as part of our NDIS services, in line with practice standards.
Clinical Supervision & Consulting
Do you offer clinical supervision?
Yes. We provide supervision for Behaviour Support Practitioners and Psychologists. Our supervision is reflective, strengths-based, and aligned with professional and NDIS frameworks.
Can organisations engage you for consulting or training?
Yes. We partner with organisations to strengthen clinical governance, deliver workforce training, and support organisational development. Contact us to discuss your needs.
General
What is your cancellation policy?
We follow the NDIS cancellation policy. Sessions cancelled with less than two business days notice may be billed at 100% of the scheduled fee.
Do you have a complaints or feedback process?
Yes. We encourage feedback and have a clear complaints process in line with NDIS Practice Standards. You can contact us directly or provide feedback via our online form.
